Eligibility For Athletic Participation
Grades 7-12
In order to be eligible on the first day of practice an athlete must turn in all required paperwork to the CVCS Athletic Office. Students will not be allowed to practice without completing requirements.
Athletic Requirements:
- Current School Year Athletic Forms Completed and Turned In
- Current Physical Exam Form/Interim Questionnaire on file
- Entering grades 7, 9, 11
- Interim years complete Interim Questionnaire Form - grades 8, 10, 12
- unless a physical has not been completed in grades 7, 9, 11
- Completed and signed Information & Consent Form IC FORM
- Completed and signed Acknowledgement of Risk Form
- Current Physical Exam Form/Interim Questionnaire on file
- Sports fee paid (or other arrangements made)
- Student is registered for school
- Student is currently academically eligible
Required Forms For Athletic Eligibility
MIDDLE/HIGH SCHOOL
All athletes must complete and turn in all required paperwork PRIOR to their first practice.
A current physical exam is required for all 7th -12th grade athletes prior to participation in practice or contests. Sports physicals are valid for two years from the date of the exam. The exception to this rule is entry into 9th grade. ALL ENTERING 9th GRADERS REQUIRE A PHYSICAL. For the off-years, an Interim Questionnaire Form needs to be completed and on file with the school.
