Transcript Request Form

  • Transcript requests will be fulfilled once final grades have been recorded.  Per the signed agreement in enrollment, no official records will be released from CVCS (including transcripts and/or report cards) until all account balances are paid in full. Requests will be processed within 5-7 business days from the day received.
